One-page medical form, issued by the California State Athletic Commission, signed twice by Muhammad Ali. Both of Ali's signatures have been scripted in blue ink, with one grading "7" and the other grading "9." This form was required to be completed by any fighter wishing to obtain a boxing license in the state of California. Ali applied on June 10, 1967, and the form notes that he received a chest X-ray and was examined by a general doctor and an eye specialist, both of whom have also signed the form. A small portrait photo of Ali, taken at the time this form was processed, is attached in the upper left corner. A stamp in the lower right notes that the Athletic Commission received the form on July 12, 1967. Ali's request for a license was ultimately denied by the Commission because he refused to be inducted into the military earlier in the year. The form (8.5 x 14 inches) displays two notebook holes along the top border, a horizontal fold, light creasing, and a few tiny edge tears at the base. In Very Good to Excellent condition overall.
